Excessive or repeated dosing can cause metabolic alkalosis, hypokalaemia, tetany, and in extreme cases seizures or arrhythmias.
Drug interactions and milk-alkali syndrome — Raising gastric and urinary pH alters absorption and excretion of many drugs including aspirin, lithium, tetracyclines, ketoconazole, iron and amphetamines — separate doses by at least 2 hours and consult a pharmacist. Combining large amounts with calcium supplements or dairy can cause milk-alkali syndrome (hypercalcaemia, alkalosis, kidney injury).
